America Publishers spotlights the release of Excruciating, a debut novel that brings the vulnerability of the operating room into a disturbing story of grief, addiction, and supernatural darkness.
A Dark New Chapter for David M. Ward
A hospital depends on trust. Patients place their lives in the hands of professionals, relying on their preparation, judgment, and vigilance. In Excruciating, David M. Ward brings readers into that familiar system of care, then begins to expose the fractures beneath its surface.
Now available, Ward’s debut medical-horror novel follows Matthew Rix, a nurse anesthetist struggling with the death of his son. His professional responsibilities continue even as grief and addiction erode his personal stability. Within an environment where concentration matters and small decisions carry consequences, his private suffering becomes increasingly difficult to contain.
America Publishers Authors Spotlight highlights this release as an introduction to Ward’s dark, character-driven fiction. Drawing on his background in nurse anesthesia and military medicine, he combines the practical detail of clinical work with psychological suspense and an emerging supernatural threat.
The result is a story that asks what happens when someone entrusted with protecting others can no longer rely on his own sense of control.
Inside the World of Excruciating
Matthew Rix works in a setting governed by routines, equipment, medications, and the expectation of professional composure. Yet the opening of Excruciating makes clear that the ordinary demands of his work have become entangled with a much more troubling personal struggle.
Since his son’s death, Matthew’s relationship with himself and the people around him has deteriorated. Everyday interactions carry the weight of anger, shame, and isolation. Colleagues continue speaking to him, patients continue needing care, and the operating room continues moving through its familiar procedures. Internally, however, he is becoming less able to manage what he carries.
Addiction adds another layer of danger. His access to medications places temptation within the same environment that demands his vigilance, creating tension between professional responsibility and compulsive need.
As the story develops, disturbing experiences begin to challenge Matthew’s understanding of what is happening around him. The hospital becomes a place where personal vulnerability, manipulation, and supernatural menace intersect. Readers enter this uncertainty alongside a protagonist whose judgment is already under strain.
Without revealing its central discoveries, Excruciating moves beyond the fear of a single medical emergency. Its suspense grows from the possibility that the danger reaches into the relationships and institutions Matthew has taken for granted.
When Clinical Control Begins to Fracture
The operating room gives Ward’s medical-horror novel a particularly unsettling foundation. It is a place where a person voluntarily relinquishes consciousness, trusting that trained professionals will remain attentive throughout an experience the patient cannot oversee.
Ward builds tension through the details of that responsibility. Medication preparation, monitoring equipment, conversations among staff, and the sounds of a working surgical environment establish the routines surrounding Matthew. These details make the setting tangible before the supernatural elements begin to disrupt it.
That familiarity matters. When something disturbing occurs in a carefully controlled environment, it challenges the assumptions that make the space feel safe. An apparently ordinary room can become frightening when one person perceives something that others do not appear to notice.
Matthew’s grief and substance use complicate this uncertainty. Readers must consider his experiences while recognizing the pressures affecting his perception and behavior. His clinical knowledge gives him a framework for understanding the body, but it cannot readily explain everything he encounters.
This meeting of medical realism and supernatural horror gives Excruciating its central tension: the procedures remain recognizable even as the world around them becomes harder to comprehend.
Grief, Addiction, and the Possibility of Redemption
Beneath the novel’s escalating danger lies a sustained concern with suffering and the ways people respond to it.
Matthew’s bereavement shapes his inner life from the opening pages. His son’s absence affects how he receives kindness, how he interprets other people’s intentions, and how he speaks to himself. Grief is present in the ordinary moments that continue after loss, including those when the rest of the world expects a person to function.
Addiction becomes part of his attempt to navigate that pain, while introducing further secrecy and risk. The novel places these pressures alongside questions of accountability. Matthew’s suffering helps readers understand his vulnerability, but his choices still have consequences for himself and others.
Manipulation deepens the threat. Characters’ desires, resentments, and weaknesses become points through which others can gain influence. As psychological suspense develops into supernatural horror, the story continues returning to the human conditions that make such influence possible.
Ward also explores the difficult relationship between suffering and redemption. The prospect of change exists within a world shaped by destructive decisions, damaged trust, and the need to confront what has happened. That struggle gives the novel an emotional thread running beneath its darker events.
A Creative Life Shaped by Medicine and Military Service
David M. Ward brings substantial professional experience to the clinical world of Excruciating. A retired U.S. Army Reserve colonel and Certified Registered Nurse Anesthetist, he has more than two decades of operating-room and trauma-care experience.
His 37-year military career began in 1988 as an enlisted combat medic. He subsequently served for more than ten years as an Air National Guard flight nurse and later spent 21 years as an Army Reserve nurse anesthetist before retiring in December 2025. Across that career, his responsibilities included clinical practice, leadership, and training.
These experiences provide relevant context for a novel concerned with vigilance, vulnerability, and decisions made under pressure. Ward understands the professional environment in which his protagonist works, including the concentration required when another person’s safety depends on sustained attention.
His fiction develops that knowledge into an imagined story exploring loss, resilience, and human behavior. The medical detail supports the setting, while the characters and supernatural elements carry the novel into darker territory.
Born and raised in Philadelphia, Ward now lives in Bucks County, Pennsylvania. Excruciating marks his debut as a novelist and introduces readers to his interest in the thin line between suffering and redemption.
